There are five different ways by which a bowler can get credit for a
dismissal - bowled, caught, leg-before-wicket, stumped and hit-wicket.
No bowler has ever achieved all five in a Test innings or even in a
match. Overall, four different dismissals by a bowler in a Test
innings are fairly common.

A. Bowlers who have achieved four different forms of dismissals in a
Test innings. The list is more or less in chronological order. (in
cases where the bowler has performed this feat more than once, the
number of instances is listed in the parentheses):

Just to add to the statement "No bowler has ever achieved all five in
a Test innings or even in a
 match" .
Harry Trott did 4 out of 5 in a match.Nothing unusual ,except that the
missing dismissal was caught.He had a stumped,hit wicket,bowled and
lbw against his name in this match http://www.cricinfo.com/ci/engine/match/62441.html
.He is the only one with caught as the missing dismissal amongst the
possible 5

now I realized that I had forgotten to include the ODI data in the
original post.
