I know the bowler types are rather subjective but here are my definitions:

Fast - averages 140 kph+
Fast Medium - averages between 130 and 140 kph
Medium Fast - averages between 120 and 130 kph
Medium - averages between 110 and 120 kph
Slow - averages below 110

Whoever first decided that Medium Fast is slower than Fast Medium did not
have a great facility in the English language. The first word qualifies the
second. Fast Medium means medium but at the higher end of medium. Medium
fast means in the middle of the fast spectrum. The categories should be
based on a contraction of the following:

Fast Fast
Medium Fast
Slow Fast
Fast Medium
Medium Medium
Slow Medium
Fast Slow
Medium Slow
Slow Slow

These can be suitably contracted thus:

Fast - fastest delivery above 150 kmph
Medium Fast - fastest delivery above 140 kmph
Fast Medium - fastest delivery above 130 kmph
Medium - fastest delivery above 120 kmph
Slow Medium - fastest delivery above 110 kmph
Medium Slow - fastest delivery above 100 kmph (spinners like Kumble)
Slow - fastest delivery below 100 kmph (mainly spin)

Another way to look at the above categories is in terms of average speeds.

Surely medium fast would be faster than fast medium.

I think when they say Fast Medium they mean that the bowler bowls more fast
deliveries than Medium ones and vice versa for Medium Fast.

From the linguistic point of view you are correct. It just so happens that
in cricket the convention usage is the reverse of what a non-cricketer would
conclude.

He was fast on that day. The main issue is what the highest speed is on a
given day or during a given period.

Hardly. If Lee bowls several slower balls to fool the batsman and some
medium pacer bowls only at his fastest, the latter's average speed might be
higher. That does not make him a fast bowler.

The average is the *worst* statistical method in this case. A modal range
would be more appropriate.
