One common cause is getting your hands too high or too "vertical"
at the top.  Try this:  have someone stand behind you (so you hit
away from them) and hold a shaft out in front of them.  Have them
sight the shaft so draws a line connecting the ball and the base of
your neck.  This is "the proper" swing plane.  Most people who
consistently hit the ball off the toe will have their hands above
or to the right of this shaft sighting.  For a good illustration
of this, see Ben Hogan's book, I think it's called "5 Lessons,
the Fundamentals of teh Golf Swing".  That's close to the title
anyway.
