From the responses to my first questionnaire, there is clearly a lot of
interest and information out there in ***land.  In this posting, I'm
incorporating several helpful questionnaire-design suggestions.  

What I'm trying to pull together is what you have found to actually work
best for you, not theories or formulas.  

When making comments related to the wind speed rows, head them with
Beaufort number at end of questionnaire (e.g, Bft#4:  Equipe II with 17.5"
fin) to make compilation easier.
